Melatonin: does it help you sleep, and is it safe?

A little. In a 2013 meta-analysis of 19 studies, melatonin helped people fall asleep about 7 minutes faster and sleep about 8 minutes longer [1], and the American Academy of Sleep Medicine suggests doctors not use it for chronic insomnia in adults [3]. The evidence is better for jet lag [5]. Labels are often wrong [6] [7], children get into it [8], and its legal status varies: in Saudi Arabia, more than 2 mg a day or any medical claim makes it a registered health product [11].

Does it help you sleep?

Melatonin is a hormone your body makes; it rises in the dark and helps set the sleep cycle [17]. As a supplement, the effect is small. A 2013 meta-analysis of 19 studies with 1,683 people found it shortened the time to fall asleep by 7.06 minutes and lengthened sleep by 8.25 minutes on average, with slightly better sleep quality [1]; the study was partly funded through Eli Lilly research awards held by one of its authors [1]. A 2023 meta-analysis found the prolonged-release form cut the time to fall asleep by about 6 minutes, whether people reported it or it was measured [2].

The American Academy of Sleep Medicine's 2017 guideline suggests that "clinicians not use melatonin as a treatment for sleep onset or sleep maintenance insomnia" in adults [3]. It's a weak recommendation, based on trials of 2 mg, with very low quality evidence, and the improvement in sleep quality wasn't clinically significant [3].

In Europe, a 2 mg prolonged-release melatonin medicine, Circadin, is authorised on prescription for the short-term treatment of poor sleep in people aged 55 or over. The EMA says: "although Circadin has only been shown to have a small effect in a relatively small number of patients, its benefits are greater than its risks." [4]

Jet lag

The evidence is better here. In a Cochrane review of 10 trials, 8 found that melatonin taken close to bedtime at the destination reduced jet lag after flights crossing five or more time zones [5]. The review dates from 2002; a 2008 search found no new trials [5]. Saudi Arabia's Food and Drug Authority lists easing jet lag and shortening the time to fall asleep as its uses [17].

What's really in the bottle

In the US, melatonin is sold as a dietary supplement, so it isn't approved by the FDA the way medicines are [7] [16].

Children

US poison centres received 260,435 reports of children swallowing melatonin from 2012 to 2021, rising 530%, from 8,337 in 2012 to 52,563 in 2021 [8]. Most were accidental (94.3%) [8]. Of the 27,795 children seen at a health facility, 4,097 were admitted to hospital and 287 needed intensive care; five needed mechanical ventilation, and two, both under 2, died [8]. Gummies were the most common form in emergency visits by children aged 5 and under [9]. The American Academy of Sleep Medicine advises treating melatonin like any medicine: keep it out of children's reach, and talk to a paediatric professional before giving it [10].

In Saudi Arabia, the SFDA's classification guidance lists "Melatonin that is in concentrations equal to or lower than 2 mg/d" under food, and "Products containing melatonin with medical claims or in concentrations more than 2 mg/day" under health products [11]. Products of 3 mg and 5 mg appear on its herbal medicines and vitamins list [12]; its list of registered drugs showed no melatonin product [13]. In the EU, food health claims for melatonin require at least 0.5 or 1 mg a portion, while several member states treat it as a medicine depending on the daily dose [18]. In the UK, the NHS says melatonin supplements "are not authorised for sale in the UK and it's a prescription-only medicine." [15] We couldn't find Kuwait's rules.

Side effects and medicines

The SFDA lists headache, nausea and dizziness among the possible risks of using too much melatonin, and because it can interact with some medicines, including blood pressure and diabetes medicines and contraceptive pills, it recommends taking it under medical supervision [17]. Nothing here is dosing advice.
