Cortisol belly and cortisol cocktails: what the evidence says
Cortisol helps your body respond to stress, and stress and exercise do raise it [1] [2]. But in studies that followed people over time, stress was linked to only very small weight gains [10], and the condition in which cortisol really does put fat on the belly and face, Cushing's syndrome, is rare and most often caused by steroid medicines [1] [3]. We found no study of the cortisol cocktail drink, while in one trial, short sleepers who slept longer ate about 270 kcal a day less [23].
What cortisol does
Cortisol is made by the adrenal glands, on top of the kidneys [1]. NIDDK, part of the US National Institutes of Health, says: "Cortisol is sometimes called the “stress hormone” because it helps your body respond to stress." [1] It also helps maintain blood pressure, regulate blood sugar, reduce inflammation and turn food into energy [1]. The Gulf Health Council, the health body of the Gulf Cooperation Council states, lists the same roles [3].
It affects almost every organ and tissue in the body [2]. Levels follow a daily rhythm: normally highest in the morning and much lower by about 4 pm [2], and NIDDK notes that cortisol production normally drops just after we fall asleep [1].
What raises cortisol
- Stress and exercise. MedlinePlus, from the US National Library of Medicine, says: "Stress and exercise can raise your cortisol levels" [2]. Pregnancy, heat and cold, and some medicines, such as birth control pills, can change levels too [2].
- Steroid medicines. The most common cause of abnormal cortisol levels is taking steroid medicines for a long time, or stopping them suddenly [2].
- Caffeine. In a trial of 48 young men and 48 young women, caffeine raised cortisol; in those who had it every day the rise was smaller, but it didn't disappear [4]. See our caffeine guide.
- Short sleep, perhaps. In young men, a night of partial or no sleep raised cortisol the following evening by 37% and 45% [5]. But a 2024 review of 24 studies found no overall difference in cortisol after acute sleep loss, except in the studies that measured it in blood [6].
- Some conditions. Depression, anxiety, heavy drinking, poorly controlled diabetes and obesity can raise cortisol off and on, without the long-term effects of Cushing's syndrome [1].
Saudi Arabia's Ministry of Health ties stress to eating: acute stress can kill your appetite, "But the secretion of the hormone cortisol during chronic stress can cause cravings for fat and sugar." [7]
Does stress cause belly fat?
On social media, high cortisol gets the blame for tiredness, a puffy face, waking at night and weight gain around the middle, the so-called "cortisol belly"; many of the accounts posting about it belong to self-described hormone or weight-loss coaches without medical qualifications [8].
Studies do find a link, but a weak one that may run both ways:
- A snapshot. Among 59 women, those with more fat around the middle released more cortisol during lab stress tests [9]. They were measured once, so the study can't show which came first [9].
- Over time. A review of 14 cohorts that followed people over time found no significant link between stress and later weight or body fat in 69% of analyses; pooled, the link had a correlation of 0.014, which the authors called very small [10]. That explains about 0.02% of the differences in weight gain (calculated by squaring the correlation).
- In hair. Across 146 groups with 34,342 people, cortisol and cortisone stored in hair were higher in people with bigger waists and higher BMIs [11]. The strongest correlation, 0.18 for cortisone and waist size, explains about 3% of the differences in waist size (calculated). The authors say their data can't show cause and effect: higher cortisol may contribute to belly fat, or reflect it [11].
- Fat makes cortisol. A 2015 review found that fat cells in obesity clearly produce more cortisol, while the findings on cortisol in the blood were inconclusive [12].
The Endocrine Society's guideline on diagnosing Cushing's syndrome adds that cortisol in the blood is usually normal, or slightly low, in obesity, though severe obesity can raise the amount in urine [13]. Stress can still affect weight through eating: the Gulf Health Council lists weight gain or loss among its effects [14], and Saudi Arabia's health ministry links it to cravings [7]. Cushing's syndrome: when cortisol really is the cause
NIDDK defines it: "Cushing’s syndrome is a disorder that occurs when your body makes too much of the hormone cortisol over a long period of time." [1] When the body makes the excess itself, it is rare: estimates range from about 40 to 70 people per million, or 4 to 7 per 100,000 (calculated) [1]. The most common cause is long-term, high-dose use of glucocorticoids, cortisol-like medicines used for conditions such as asthma, rheumatoid arthritis and lupus [1].
People with very high cortisol for a long time tend to have clear signs: weight gain, thin arms and legs, a round face, fat at the base of the neck and between the shoulders, easy bruising, wide purple stretch marks and weak muscles [1]. The Gulf Health Council's list includes weight gain mostly around the belly, and a round, red, puffy face [3].
No single sign proves it. The Endocrine Society's guideline says the features that best distinguish Cushing's are easy bruising, a red face, weakness in the upper arms and thighs, and reddish-purple stretch marks wider than 1 cm, while weight gain, obesity and a full face are common in the general population [13]. It recommends testing mainly people with several features that are getting worse, or signs unusual for their age, and advises against wider testing [13]. The guideline dates from 2008 and took no company money, and a 2022 study judged its advice still valid [13] [15]. In that study, of 377 people referred to German centres with suspected Cushing's, obesity or weight gain was the main reason for referral in 30% of those who turned out not to have it, against 4% of those who did [15].
If you take steroid medicines, the Gulf Health Council advises: "If you are prescribed steroid-containing medications, ask your doctor to monitor your cortisol levels." [3] And from MedlinePlus: "But don't stop using any medicines without talking with your provider first." [2]
Cortisol face
The claim: stress raises cortisol, which makes your face round and bloated, and people sell products and programmes promising a slimmer face by lowering it [16].
A round face is a sign of Cushing's syndrome, but alongside other signs, in people with very high cortisol for a long time [1] [3]. On its own, a full face is one of the less telling features [13]. The guideline lists physical stress, such as being in hospital, surgery or pain, among conditions that can raise cortisol but are unlikely to cause any of its features [13], and NIDDK says: "Some people have high cortisol levels off and on but don’t develop the long-term health effects of Cushing's syndrome." [1] A single cortisol test won't settle it either: "A cortisol test alone can't diagnose the cause of abnormal cortisol levels." [2] The Gulf Health Council's advice: if you suspect Cushing's syndrome or have any of its symptoms, see your doctor [3].
The cortisol cocktail
Also called an adrenal cocktail, the drink is mostly orange or lemon juice, coconut water and a pinch of sea salt, often with cream of tartar; users claim it reduces stress and even helps with weight loss [17]. Influencers also say it lowers stress hormones, boosts energy, balances the adrenal glands and treats what they call adrenal fatigue [18].
We found no study of the drink. The Endocrine Society says the adrenal glands don't get "adrenal fatigue" or lose function because of mental or physical stress [19]; a 2016 systematic review of 58 studies found nothing to show that adrenal fatigue is a real medical condition [20].
What's in the glass still counts. WHO says of fruit juice that "most varieties including those without added sugars contain significant amounts of free sugars" [21], and recommends that adults have less than 5 g of salt a day [22]. What reliably helps
- Losing fat overall. Our belly fat guide covers what the trials found about shrinking belly fat.
- Enough sleep. In a US trial, 80 adults with overweight who usually slept less than 6.5 hours got a counselling session on sleep habits aimed at 8.5 hours in bed, then followed it at home for 2 weeks: they slept about 1.2 hours more a night, ate about 270 kcal a day less and weighed 0.87 kg less than the control group [23]. - Regular activity. Saudi Arabia's Ministry of Health says: "Regular physical activity not only improves sleep; but also directly combats stress." [7] In a 2018 review, fitter or more active people had a smaller cortisol rise in a standard lab stress test in 7 of 12 studies, though most of the studies were snapshots [24].
- Stress programmes, with modest expectations. In a small trial, 47 women with overweight or obesity either took a 4-month mindfulness programme for stress eating or waited: the groups didn't differ in cortisol on waking, weight or belly fat [25]. The Gulf Health Council's advice on managing stress includes enough sleep, a balanced diet and regular physical activity [14].

We found no study of it. Adrenal fatigue, which it is said to treat, isn't a recognised condition, and the drink is mostly juice, coconut water and salt, so its sugar and salt count towards your daily limits.
What actually helps with stress and belly fat?
Of the options here, sleep and activity have the most support. In one trial, short sleepers who slept about 1.2 hours more ate about 270 kcal a day less, and Saudi Arabia's Ministry of Health says regular physical activity improves sleep and directly combats stress.
